The New Jersey Early Intervention System (NJEIS) is designed to address developmental delays or disabilities for infants and toddlers up to age three. When a child is referred to NJEIS the county’s Targeted Evaluation Team (TET) will conduct an evaluation to determine if the child is eligible for NJEIS services.
Once a child is deemed eligible for Early Intervention (EI) services, the service coordination unit begins working with the family to create an Individualized Family Service Plan (IFSP) that identifies and meets the needs of the children and their families. The service coordinator becomes the family’s point of contact throughout the EI process. The service coordinator is responsible for ensuring appropriate services are in place to adequately address the specific needs identified for each child during the IFSP. Using a multi-disciplinary approach, the service coordinator works closely with the families and practitioners to continuously monitor the child’s progress with implemented services by conducting periodic review meetings to track and adjust identified goals and outcomes. In addition, the service coordinator assists with the transitional planning of children aging out of NJEIS and entering into the public education system. Under Part C of the Federal Guidelines and NJEIS polices and procedures, the service coordinator ensures the eligible child and child’s family receives rights, procedural safeguards and services authorized under the NJEIS.
The service coordination unit is comprised of professionals from various backgrounds and experience in early childhood development. The team services all NJEIS eligible children in Passaic County and services over 1,000 children per year.
