" | | Program Description | | Congregational Health Ministry is a program sponsored by Catholic Charities in the Diocese of Paterson, St. Clare's Hospital, Dover, St. Mary's Hospital, Passaic and St. Joseph's Regional Medical Center, Paterson and Wayne. Our mission is to address the health needs of our communities through services that promote, restore and maintain well being. Our goal is to facilitate implementation of Congregational Health programs. Congregational Health Ministry provides a Liaison to initiate and support Congregational Health programs in faith groups within the Diocese of Paterson and the surrounding areas. The Congregational Health Ministry Liaison will work closely with the faith community to begin a Health Ministry program. A Health Ministry program involves a pastor who is committed to the concept, a leader, usually a nurse and a health committee, a volunteer group who are willing to work on the goal of improving the health of the Congregation. The Congregational Health Ministry Liaison works with the leadership group to plan and implement an individualized health ministry program. The link of a health ministry can bridge the gap between professional health care providers and the community. A Congregational Health Nurse can assist families to navigate the health care system, access care, obtain accurate information for disease management and connect to services for the elderly, chronically ill and medically underserved. The faith community can be a vehicle for wellness. A health ministry program is able to provide: - Screenings
- Education and counseling
- Referrals to resources, advocacy
- Volunteer training and coordination,
- Empowerment of lifestyle choices.

     The Congregational Health Ministry Liaison will link the health ministry to the support and services of the collaborating hospitals. | | Who is eligible? | | Congregational Health Ministry services are available to any faith-based congregation within the service areas of the partnering hospitals.
